Located in Poblacion Weste, Santa Cruz, Ilocos Sur, the health center is a primary care facility serving the community with outpatient care, emergency services, laboratory and radiology services, and public health programs for families and individuals.

Every ITR follows this route from arrival to release — E-LOCATE shows you which step it's on.
Blood pressure, temperature, pulse rate, height, weight, and BMI are recorded.
History taking and consent signing, along with retrieval of the patient's existing record.
Registration, PhilHealth check utility, patient identity confirmation, and PhilHealth PIN validation.
Current consultation is encoded and the mutual care agreement is signed.
Physical examination, risk assessment, diagnosis, and management with the attending physician.
If ancillary services are needed: laboratory work, diagnostics, and medicine dispensing.
If ancillary services are neededIf no ancillary services are needed, the patient proceeds directly to health education.
If no ancillary services are neededReferral to another facility or specialist, if necessary.
Final documentation of the consultation, diagnosis, and services rendered.
