Patient ITR Locator & Scheduling System

Last updated: October 2026

Terms of Use
1. Acceptance

E-LOCATE is a Patient ITR Locator & Scheduling System run by the hospital operating E-LOCATE. By creating an account or logging in, you confirm that you have read and agree to these Terms of Use and the Privacy Policy below. If you do not agree, please do not use the system.

2. Your account
  • Use your own registered Gmail account and keep your password private.
  • You are responsible for everything done under your account.
  • Personnel / staff accounts must be approved by the admin before they can be used.
  • Tell the administrator right away if you think someone else has used your account.
3. Proper use
  • Use E-LOCATE only for its intended purpose: locating ITRs and tracking visit or schedule status.
  • Do not look up, copy, share, or change records you have no authority or need to access.
  • Do not try to break into the system, bypass its controls, or interfere with other users.
  • Patients may only view information that belongs to them.
4. Accuracy of information

E-LOCATE helps you find where a record is and what its status is. It is a guide only and does not replace the hospital staff, official records, or professional medical advice. If something looks wrong, please confirm it with the hospital personnel.

5. Suspension

The admin may suspend or remove an account that breaks these terms or puts patient information at risk.

6. Changes

These terms may be updated from time to time. Continuing to use E-LOCATE after an update means you accept the new version.

Privacy Policy
1. What we collect
  • Account details: your full name, Gmail account, role (patient, personnel, or admin), and office designation for personnel.
  • System records: ITR and visit or schedule information needed to run the service.
  • Login activity: every time you log in (including when a new patient account signs you in automatically after registration), the system records the date and time, your IP address, your approximate location, and details about your device (such as device type, operating system, and browser).
  • Precise location: when you open the login or patient registration page, your browser asks permission to share your current location. If you allow it, your location at the time of login is saved in the activity log. If you block it, you can still log in, and the system will use an approximate location based on your IP address instead.
2. Why we collect it
  • To verify who you are and give you the right access.
  • To find ITRs and manage visits and schedules.
  • To keep an activity log that protects patient records and shows who accessed the system and when.
  • To detect and investigate unauthorized or suspicious access.
3. Who can see it

Your information is available only to authorized hospital personnel and administrators who need it for their work. Activity logs are visible to the admin. Your information is not sold or used for advertising.

4. How we protect it

Accounts are protected by passwords and role-based access, so each user only sees what their role allows. No system is completely risk-free, so please log out on shared devices and never share your password.

5. How long we keep it

Records and activity logs are kept only as long as needed for hospital operations, security, and any legal or record-keeping requirements.

6. Your rights

Under the Data Privacy Act of 2012 (Republic Act No. 10173), you have the right to be informed about how your personal data is used, to access it, to correct inaccurate information, and to object to or ask for the removal of your data, subject to the hospital's legal duties to keep medical records. Health-related information is treated as sensitive personal information and handled with extra care.

7. Questions or requests

To ask about your data or make a request, please contact the E-LOCATE system administrator.
